Player Application
Please fill out one application per player. You can print out the application and fill it in by hand OR download the PDF "form" and fill it in on your computer then print it out.

Volunteer Application
All Little League volunteers must complete a volunteer application and undergo a background check.  Please complete this form and return to a team manager or other league official.

Fairgrounds Waiver
One Fairgrounds waiver is required per player.

Santa Cruz County Fairgrounds Waiver

Proof of Residency
Parents are required to present three forms of documentation to establish residency OR complete a school enrollment form.

*IF* you live outside the PVLL Boundaries, BUT the player(s) you are signing up attend a school *INSIDE* our boundaries, then you may play for PVLL by completing the school enrollment form.  This form replaces the 3 proofs of residency requirement. You will need one per player that fall into this category. Players with home addresses within our league boundaries who also attend school inside our boundaries can also use this form. This form only needs to be completed once per player until they switch schools.
